By typing my initials below, I acknowledge that a 2026-27 Medical Form and Student Covenant must be completed before my youth is allowed to participate in this event. Forms are available here. Additional forms might also be required. I acknowledge that there will be a $50 late fee for any paperwork turned in after published deadlines. 

By typing my initials below, I acknowledge that all payments for this event should be considered non-refundable. Trip deadlines and fees are carefully considered based on policies of ministry partners, and when registrations are cancelled, the church is required to make payments regardless. Partial refunds may only be administered if the church can do so without incurring a loss. Furthermore, I acknowledge that if paying by check, it is also due by the registration deadline. 

I acknowledge that in order to serve as an adult leader, I must complete Safe Sanctuaries and a Background check. I either already fulfill those two requirements or agree to do so by the designated deadline. 

I acknowledge that in order to drive one of the church shuttles or a van rented by the church, I must email a copy of my driver's license to childrebrand@firstcary.com to be added to the church insurance. I have already done so or will by the designated date.
